(Click on the Employee Testimonials link above to see why our team members love working at AMPLIFY.) Eligibility Team members who work 38 hours or more per week are eligible for all of our benefits. New team members become eligible for our benefits (including medical/dental) on the first of the month following three full months of employment. So, if you start on 2/1 and you work 38 hours or more per week, then you are eligible for benefits on 5/1. If you start after the first of the month, then you become eligible for benefits a month later (6/1, in this example). Medical We are insured by Humana (www.humana.com) and offer two levels of PPO health insurance plans. Dental Our dental insurance is provided through Guardian and we offer three plans: two Dental PPO plans and a Dental HMO plan.
Dental PPO - NAP: With this plan, you may see any dentist you wish. The calendar year deductible is $50 for an individual/$150 for family. The maximum benefit per eligible family member is $2,000 per year. Diagnostic and preventive services are covered at 100%, no deductible. Basic restorative services are covered at 80% after deductible and major restorative services are covered at 50% after deductible. Orthodontia is available to children only and is covered at 50% (no deductible) up to a lifetime maximum of $2,000.

Dental PPO - Value: Like the PPO-NAP plan, the PPO - Value plan allows you to see any dentist you like. However, you will pay less out of pocket if you visit an in-netowrk dentist. Deductibles and maximums are the same as the PPO-NAP plan. Basic care is covered at 100%, major care at 60%. Premiums: Refer to the Premiums chart above. Premiums are deducted on a pre-tax basis, lowering your taxable income.
Dental HMO: With this plan, you must see a dentist who is in the plan. If you use a dentist who is not on the plan, no benefits are payable and you are responsible for the entire expense. In network, there are no deductibles, no claim forms to file, no referrals required for specialty dentistry services, and no annual maximum. (View dental providers at http://www.geoaccess.com/guardian/po56/SearchCriteria.asp . Select "DHMO/Pre-paid",enter your zip code, and click "Continue". Choose "Managed Dental Guard" for the plan.) With DHMO, you just pay your monthly premium, plus at the time of service you pay a copayment. Premiums: Refer to the Premiums chart above. Paid Time Off Amplify offers a Paid Time Off program instead of vacation and sick time plans. Paid Time Off gives our team members the flexibility to use their time for what is important to them. PTO can be used for vacation time, sick time, or "I just want a day for me" time. PTO starts accruing the first pay period you are an employee. The accrual schedule is below. NOTE: Days Accrued Annually and Hours Accrued Annually are based on a 40 hour work week. These amounts will differ for part-time employees. Other Benefits In addition to the basic benefits, we also offer: Medical and Dependent Care Flexible Spending Accounts, which allow you to deduct money on a pre-tax basis to pay for medical expenses that your insurance doesn't cover or dependent care expenses (note: FSA benefits are only available to employees working 38 hours or more per week); 401(k) Plan* with employer matching of 100% up to the first 1% contributed by the participant, and 50% for up to the next 5% contributed by the participant (total match of 3.5% on participant's 6% contribution); Defined benefit (pension) plan* that is fully funded by the credit union, providing an additional source of retirement income for eligible team members; Supplemental benefits, such as supplemental life insurance and cancer/critical illness insurance; Voluntary benefits* such as pet insurance, prepaid legal services and identity theft protection; Paid Holidays*; Credit Union membership* and access to services such as BillPayer at no charge; Tuition Reimbursement Program ($2,000 per calendar year for undergraduate studies; $5,250 per calendar year for graduate studies)*; Recognition Program*; Discounted membership to Gold's Gym*; Company gym at Parmer offices*; Up to 40 hours per year of paid time off to perform community service during work hours (20 hours for part-time employees)*; and Wardrobe Assistance Program* (provides an interest-free loan of up to $500 for the purchase of professional clothing/shoes or AMPLIFY logo wear, with an additional $250 available to purchase work out clothing as part of our wellness program)
